This Informed Consent Form is required by GDPR for the processing of your biosamples and the storage of the derived biodata (such as DNA, RNA, Microbiome datasets) by our Laboratory LAB NAME. Please read the following information carefully and provide your consent by checking the relevant boxes and submitting the form.

What is DNA?

DNA contains genetic information that acts as a blueprint for how parts of your body are made and work, determining such things as eye and hair color. DNA is made up of long strands of repeating letters that form a code. These letters, A, C, T and G represent individual chemical units that form DNA. The order in which these letters are written is very precise. In the same way that a spelling mistake in a word could change its meaning entirely (for example, mean and meat), a single change in the sequence of DNA can lead to a change in how the body works, which may lead to disease or influence how you respond to medication.

What is genetic testing?

Genetic testing involves taking your DNA sample and determining the sequence of your genetic code. Changes in your genetic code may help your healthcare team choose the best course of treatment for you.

Consent for Biosample Processing and Biodata Storage:

I hereby give my informed consent to the LAB NAME for processing my biosamples and storing my derived biodata (such as DNA, RNA, Microbiome datasets). I understand that the laboratory will store my biosample and biodata until the corresponding BioNFT by GenoBank.io is properly revoked using my corresponding BioWallet.

Risks:

I understand that there are potential risks associated with the processing and storage of my biosamples and biodata. These risks may include, but are not limited to, the unintentional disclosure of personal genetic information, potential emotional distress due to learning about genetic predispositions, and potential misuse of genetic data by unauthorized third parties. I acknowledge that I have read and understood these risks, and I accept them as part of my participation.

Consent for Data Protection and Privacy:

I have read and understood the LAB NAME’s data protection and privacy policy. I give my consent for the LAB NAME to process my biosample and store my derived biodata until I revoke the corresponding BioNFT using my GenoBank.io’s BioWallet in accordance with the GDPR and other relevant data protection regulations.
